Fees, Payments, Insurance

Our physicians participate in many insurance plans, but not all.  If you would like avail yourself of any insurance benefits, we suggest that you check with your insurer prior to making your appointment to see if our physicians participate in your network and to confirm what coverage is provided.

Be sure that you understand what portion of payment you will be responsible for when you come to our practice.  If you have a co-pay for specialist visits, you will need to make that payment at the time of your appointment.  Please bring cash, check, or credit card to make this payment at the time of service.  If you are without coverage for your care at our practice, you will need to arrange for payment at the time of your visit.

If your insurer requires a referral in order to cover your care at a specialist’s office, please make sure you understand how to obtain that referral (electronically or in hard copy) and have it in place (or with you) at the time of your visit.

Finally, be sure to bring your insurance card with you to the visit, as proof of coverage and so that our office staff can make a copy of the card for purposes of billing and records.

If the above items are not satisfied, you may be responsible for the charges or may need to reschedule your appointment until such arrangements are made.
